Health Notices

Health Notice for Families

The guidance for the Health Notice for Families is that you will type in the information needed in yellow and check the disease that has been in the classroom. You will then send this to the families. If you have any questions regarding a disease, you can call your local health department and they will give you more information.
 

Covid-19 Health Notice for Families

The COVID-19 Health Notice is separate from the Health Notice.  That is because the Health Department will guide you in what to share with the families.  If you have a positive case, call the health department, then complete this letter.  The health department will also guide you on if you need to close and quarantine, for how many days or are able to stay open.  Every situation will be different.  Hopefully you won’t need to use this form.  Just like the health notice, you will complete the areas in yellow and send to the families after communicating with the health department.
